Duplicate Logon Icons


Has anyone encountered this before?  The link shows exactly what is happening however it is just an image I found on the net (with no solution listed).  I am not totally Your cache administrator is webmaster. I want this issue resolved more than anybody can understand with the amount of issues it's caused internally for us. The other 4 are user accounts.
